Wikipediaप्रश्नावली (Questionnaire) प्रश्नों या कथनों का समूह हैं, जिसके माध्यम से व्यक्ति से पूछकर सूचनाएं एकत्रित की जाती हैं। प्रश्नावली, अनुसन्धान करने का एक औजार है जिसमें लोगों से सूचना एकत्र करने के लिए उनसे बहुत से प्रश्न पूछे जाते हैं। इस विधि की खोज 1998 में लन्दन सांख्यिकी सोसायटी (Statistical Society of London) ने किया था।
प्रश्नावली, निर्माण में एक आत्मनिष्ठ ( Subjective) तथा प्रयोग में वस्तुनिष्ठ (Objective) विधि है। इसका प्रयोग तब किया जाता है जब तथ्यात्मक (Factual) सूचनाओं की आवश्यकता होती है। प्रश्नावली का निर्माण इस प्रकार किया जाता है जिससे व्यक्ति के वांछित गुणों का मापन हो सके। प्रश्नावली का प्रयोग व्यक्तिगत तथा सामूहिक दोनों रूपों में किया जा सकता है। यदि प्रश्नावली का प्रयोग समूह के लिए किया जाता है तो यह समय, धन और श्रम की बचत करने मे सहयोगी होता है।
